This information is for individuals seeking licensure in the state of Texas pertaining to the Supervisor Agreement form.


When you submit your Supervisor Agreement form to be approved, the LPC-Intern will need to include a copy of their proposed supervisor's wallet card as evidence that the intern is aware of the supervisor's licensure expiration date. Failure to submit this with your application will delay the approval of your supervisor and or site.
